Introduction

The Universal Health Coverage (UHC) target is a crucial goal for many countries, including Indonesia. Achieving this target requires a significant increase in the number of people participating in health insurance programs, such as the National Health Insurance Program (JKN) or BPJS Mandiri. However, the number of BPJS Mandiri participants in Kisaran Timur District, Asahan Regency, is relatively low, with only 11,729 people registered. This study aims to explore the influence of family socioeconomic characteristics on community decisions to register as BPJS Mandiri participants.

Background

Kisaran Timur District in Asahan Regency faces challenges in achieving the UHC target in 2019. The low number of BPJS Mandiri participants is a significant concern, as it may limit access to health services for the community. Understanding the factors that influence community decisions to register as BPJS Mandiri participants is essential to develop effective strategies to increase participation and achieve the UHC target.

Methodology

This study uses a survey method with a cross-sectional study approach. The research subjects were all family heads who live in East Kisaran District, with a total of 16,843 family heads. The sample was taken using a proportioned non-random sampling and purposive sampling technique using the Slovin formula, producing 100 samples. Data was collected through interviews with questionnaires, and analyzed using the Chi-Square test and multiple logistics regression tests.

Results

The Chi-Square test results show that the work variables, health status assessments, income, and contribution prices are related to community demand to become BPJS Mandiri participants. Multiple Logistics Regression Analysis revealed that the work variable (p = 0.040), health status assessment (p = 0.029), income (p = 0.001), and BPJS fees (p = 0.035) have a significant influence on public demand to become BPJS Mandiri participants. From these results, the revenue variable has the most dominant influence with the highest Exp (B) value, which is 3.317 times, shows that the higher the income, the more likely the community to register as BPJS Mandiri participants in the East Kisaran District area.

Discussion

This analysis shows that income is a major factor that influences people's decisions to become BPJS Mandiri participants. This confirms the importance of individual economic capacity in accessing health services. When a person's income increases, they tend to be better able to pay BPJS contributions, which in turn contribute to the increase in the number of BPJS Mandiri participants.

The results of this study also highlight the importance of socialization regarding the National Health Insurance Program (JKN). Inviting people who have utilized the JKN program as a model can increase community understanding of the benefits and importance of becoming BPJS Mandiri participants. With this step, it is expected to expand the scope of BPJS Mandiri membership and support the achievement of UHC targets in Kisaran Timur District.

Conclusion

Overall, this research provides an important insight into the dynamics of socioeconomic factors that influence community decisions in participating in health programs. By paying attention to these factors, more effective strategies can be developed to increase community participation in the BPJS Mandiri program, which in turn will have a positive impact on public health in Asahan Regency.

Recommendations

Based on the findings of this study, the following recommendations are made:

Increase socialization regarding the National Health Insurance Program (JKN): Inviting people who have utilized the JKN program as a model can increase community understanding of the benefits and importance of becoming BPJS Mandiri participants.
Develop effective strategies to increase community participation: By paying attention to the socioeconomic factors that influence community decisions, more effective strategies can be developed to increase community participation in the BPJS Mandiri program.
Expand the scope of BPJS Mandiri membership: By increasing community participation in the BPJS Mandiri program, the scope of membership can be expanded, which in turn will support the achievement of UHC targets in Kisaran Timur District.

This study has several limitations, including:
