Unraveling Challenges in Management of Malnutrition at Medan Labuhan Puskesmas: Analysis of Program Implementation in 2008

Medan City, a major urban center in Indonesia, still grapples with the serious issue of malnutrition, particularly in the Medan Labuhan region. In 2007, the Medan Labuhan Health Center recorded the highest number of malnourished children under five in Medan, with a staggering 32 children affected. To address this pressing concern, the Medan Labuhan Health Center launched a malnutrition management program. However, its implementation in 2008 faced numerous challenges, which this study aims to analyze.

Background of the Study

Malnutrition is a significant public health concern worldwide, including in Indonesia. It affects not only the physical health of individuals but also their cognitive and socio-economic development. In the context of Medan Labuhan Puskesmas, the malnutrition management program was introduced to address the high prevalence of malnutrition among children under five. The program aimed to provide essential nutrition services, including growth monitoring, vitamin A supplementation, and treatment of malnutrition toddlers.

Methodology

This study employed a health system approach with a qualitative descriptive survey method. Data was collected through in-depth interviews with the leadership of the Puskesmas, Nutrition Officers, and 25 Chairman of the Posyandu Cadre in the Medan Labuhan Puskesmas working area between February and May 2009. The study aimed to identify the strengths and weaknesses of the malnutrition management program, as well as the challenges faced by the program implementers.

Results

The study revealed several weaknesses in the malnutrition management program at Medan Labuhan Health Center. In terms of input, there was a lack of nutritional staff, posyandu cadres who were inactive and less skilled, as well as limited facilities and infrastructure. In terms of process, programs that included growth monitoring, vitamin A supplementation, FE tablets for pregnant women, complementary foods, and treatment of malnutrition toddlers, had not reached the expected target. Exclusive breastfeeding coverage was still low, indicating a need for more intensive efforts to promote good parenting practices.

Main Cause Factor

The study identified the low knowledge and awareness of the public about good parenting as the main cause factor of malnutrition in the Medan Labuhan region. In addition, the lack of cadre participation in providing counseling and counseling at Posyandu was also an obstacle. This highlights the need for more intensive efforts to increase community education and participation.

Proposed Solutions

To address the challenges faced by the malnutrition management program, the study proposed several solutions:

Increasing supervision of the implementation of activities in Posyandu by the leadership and nutrition officers of the Medan Labuhan Puskesmas.
Routine training and refreshment for Nutrition Officers and Posyandu cadres, to improve their ability to provide information and counseling to the community.

Increased incentives for Posyandu cadres, as a form of appreciation and motivation to increase their participation.
Improved facilities and infrastructure at Medan Labuhan and Posyandu Puskesmas, to support the activities and participation of cadres.

Conclusion

The success of the malnutrition management program depends not only on the efforts of health workers but also on the active participation of the community. Increasing public awareness of the importance of good parenting, as well as an active role in participating in activities in Posyandu, will be the key in reducing malnutrition rates in the Medan Labuhan region. The proposed solutions aim to address the challenges faced by the program and improve the quality of nutrition services provided to the community.

Recommendations
